Accident Investigation
We investigate accidents and dangerous occurrences to ensure that they do not happen again in the future.
If any of the following have occurred you must report it:
Employee has had an accident.
Member of the public been injured at you premises and been taken directly to hospital.
Someone been killed as a result of an accident either at work or in connection with the work activity on site?

This is a legal requirement under The Reporting of Injuries Diseases and Dangerous Occurrences Regulations 2013 (RIDDOR).
How to report an Accident:-
All incidents can be reported online but a telephone service is also provided for reporting fatal/specified, and major incidents only. For these please call the HSE Incident Contact Centre on 0345 300 9923 (opening hours Monday to Friday 8.30 am to 5 pm)
There is no longer a paper form for RIDDOR reporting, because the HSE online system is the preferred reporting mechanism. Should it be essential for you to submit a report by post, it should be sent to:

Depending on the severity of the accident and the circumstances in which it happened we may contact you in order to instigate an investigation in to the probable causes of the incident, and to determine if any remedial works or changes in working practices need to be made.
