Before you start

  • The address of your second home.
  • The sale / rental status of your second home.

Please note:

  • All fields marked with an asterisk (*) must be completed.
  • Dates should all be typed in the format DD/MM/YYYY (as in 01/07/2021).
  • Extra help, where provided, can be found immediately after each field.

When you have successfully completed this form, you will receive a 16-digit reference number and should receive a confirmation email.
